To participate in athletics, students must have a current sports physical and be registered online through FinalForms (2025-26 fall sports registration began on July 1, 2025). Students can get sports physicals from their primary care provider or at the La Clinica School-Based Health Center at Phoenix High School.

All student-athletes must have a current sports physical uploaded to FinalForms before participating in any athletic activities.

Please ensure the following:

• The physical must be completed using the OSAA-approved form.
• The “History Form” portion of the physical is digital in FinalForms or printable from the PHS website.
• The “Physical Examination Form” portion must be signed and dated by a licensed medical provider.
• Sports physicals are valid for two years from the date of the exam.
• If your athlete had a physical exam in Arbiter, the expiration date will transfer to FinalForms automatically.

Note: The PHS and TMS athletics office does not keep hard copies of physicals. All records are stored electronically in FinalForms. Parents are responsible for uploading and maintaining a current physical for their athlete in the FinalForms portal.  

Physicals may be obtained from your family physician or at one of the following locations:

• La Clinica Student Health and Wellness Center at Southern Oregon University (541-494-4875)
• La Clinica School-Based Health Center at Phoenix High School (541-897-5151)
• Providence Sports Medicine - Stewart Meadows Medical Plaza (541-732-8790)

Health insurance is required but not provided by the district. Student accident insurance is available to purchase if you are not already insured or want additional insurance coverage. An enrollment form for insurance is available in the TMS Athletics Office.

Please remember the TMS Office is closed from June 18 until August 19.  All calls will be returned after that date.
